This paper presents the stylized WageIndicator questionaire. The word 'stylized' implies for instance:

• The stylized questionnaire includes the most important questions, and does not show all questions and features, because the paper would otherwise count hundreds of pages.
• The stylized questionnaire shows the routing for employees only and not for other employment status groups.
